What to put on a fax cover sheet

The receiving office uses the cover sheet to route your pages to the right person. Keep it short and complete.

  • Who it's for: a person or department, not just the company name.
  • How to reach you: a phone number, and a fax number if you expect pages back.
  • Page count, including the cover, so they can tell if something is missing.
  • A reference such as a case, claim or patient account number if you have one.
